I am the parent, legal guardian, or authorized adult responsible for the participant(s) listed above. I voluntarily allow the participant(s) to take part in hands-on creative activities, workshops, studio sessions, private events, birthday parties, and related experiences at Atlas & Hands.
I understand that Atlas & Hands activities may involve arts, crafts, tools, materials, and studio equipment, including but not limited to beads, small objects, jewelry components, charms, glue, hot glue, cream glue, scissors, cutting tools, paper, fabric, textile patches, heat transfer materials, fuse beads, irons or heating tools operated by staff, soap-making materials, candle or seasonal materials, fragrances, dyes, molds, containers, decorative pieces, paints, markers, and other craft supplies.
I understand that participation may involve inherent risks, including but not limited to minor cuts, scrapes, burns, skin irritation, allergic reactions, contact with small parts, choking hazards for younger participants, slips, trips, falls, spills, damage to clothing or personal belongings, and other risks associated with hands-on creative activities.
I agree that the participant(s) will follow staff instructions and use materials and tools responsibly. I understand that Atlas & Hands staff may modify, restrict, or stop participation if a participant's behavior creates a safety concern for themselves, other guests, staff, or the studio.
I agree to inform Atlas & Hands of any allergies, sensitivities, medical conditions, behavioral concerns, or special needs that may affect participation. This includes but is not limited to fragrance sensitivity, skin sensitivity, latex allergy, food allergy, contact allergy, asthma, sensory sensitivity, or any condition that may require special attention.
I understand that Atlas & Hands is a creative studio and activity space and does not provide medical care, childcare, daycare, or one-on-one supervision unless specifically agreed in writing.
In the event of illness, injury, or emergency, I authorize Atlas & Hands staff to contact emergency services and/or seek emergency medical assistance if I cannot be reached immediately. I understand that I am financially responsible for any medical care or emergency services provided.
To the fullest extent permitted by California law, I release and hold harmless Atlas and Hands LLC, Atlas & Hands, its owners, managers, employees, contractors, instructors, volunteers, agents, landlords, and affiliates from any and all claims, liabilities, demands, losses, damages, costs, or expenses arising out of or related to participation in Atlas & Hands activities, except in cases of gross negligence or willful misconduct.
I understand that personal belongings are brought into the studio at my own risk. Atlas & Hands is not responsible for lost, stolen, damaged, or stained personal items, clothing, accessories, or finished projects.
I understand that projects may include small parts, decorative objects, fragrances, dyes, hot materials, adhesives, or other supplies that may not be suitable for all ages. I accept responsibility for determining whether the activity is appropriate for the participant(s).
I understand that this waiver applies to today's visit and future participation at Atlas & Hands unless revoked in writing or replaced by a newer waiver.
By typing my name and submitting this form, I agree that my typed name and electronic submission constitute my electronic signature and have the same legal effect as a handwritten signature.
